This notice describes how personal and health information may be used and disclosed and how you can access it. Living Well IVs limits collection and use to what is reasonably necessary to provide services, operate the practice, process payments, and meet legal obligations.
Information we collect
- Identity and contact information you provide.
- Appointments, intake answers, consents, care instructions, and staff notes.
- Communication preferences and SMS/email delivery history.
- Payment status and transaction references; card details are handled by Stripe.
- Security and audit data necessary to protect your account.
How information is used
We use information for treatment-related operations, scheduling, secure communication, payment, quality improvement, legal compliance, and other purposes permitted by law. We do not sell patient information.
If you separately opt in to optional marketing, we share only the contact information needed to manage newsletters and unsubscribe requests with our email marketing provider. We do not send appointment, purchase, service, intake, or clinical details to that marketing list.
Your choices and rights
- Request access to, amendment of, or a copy of eligible records.
- Request restrictions or confidential communications where legally available.
- Withdraw optional marketing consent without affecting treatment communications.
- Opt out of SMS by replying STOP and restore messages by replying START.
- Request an accounting of certain disclosures.
Safeguards and vendors
We use access controls, encryption, audit history, private storage, and contracted service providers to protect information. No system can guarantee absolute security. Please do not send sensitive health information through the public contact form.
